1. Complete the Nomination and Statement of Faith form along with their Member Advocate. This includes agreeing to and signing the 6:21 Experience Statement of Faith that is described in the Statement of Faith section of this page. The nomination form is available in the download section of this page. The Nomination and Statement of Faith form is due at the March meeting.
2. If selected to apply for the annual grant, the ministry must complete and submit the Grant Application Form and documents requested in the Application Form. The Application and supporting documentation is due two weeks prior to the April meeting. The Application form is available in the download section of this page.
3. Prepare a 10-minute presentation for the April meeting that shares your mission and how you intend to use the grant with the 6:21 Experience members.
4. Along with your Member Advocate, set up 2 site visits for 6:21 Experience members in May, June and July. Host these site visits, sharing more information about your mission and answering member questions.
5. If awarded an annual 6:21 Experience Grant, join members in October to celebrate your grant award and the goodness of God.

1. We believe that the entire Bible is the inspired and inerrant Word of God, the only infallible rule of faith and practice.
2. We believe that there is only one God, eternally existing in three persons: Father, Son and Holy Spirit.
3. We believe that Jesus Christ is the only begotten Son of God, conceived by the Holy Spirit, born of the Virgin Mary, and is true Man and is true God.
4. We believe that in this age, God manifests Himself through the Holy Spirit, who is a distinct personality, eternally coexistent with the Father and the Son through His indwelling presence.
5. We believe that God created man in his own image, male and female.
6. We believe man sinned and thereby incurred not only physical death but also spiritual death which is eternal separation from God and is upon all mankind.
7. We believe that the Lord Jesus Christ died for our sins according to the Scriptures, as a representative and substitutionary sacrifice, and all who believe in Him are justified on the basis of His shed blood.
8. We believe that the Lord Jesus Christ was resurrected in body, ascended into Heaven, presently lives there for us as High Priest and Advocate, and that His personal, physical return to earth is imminent.
9. We believe that every believer in the Lord Jesus Christ must obey Christ and his teachings and pursue righteousness.
10.. We believe that the Lord Jesus Christ commanded his followers to make disciples of all peoples throughout the entire world and witness to the truth and love of God.
11. We believe that there will be a bodily resurrection of both the just and unjust, the just to the everlasting blessedness of the saved.
The Nomination and Statement of Faith Form is to be completed by the 6:21 Advocate and Ministry. This form is due prior to the March Nominating meeting. The ministry representative must review and sign the Statement of Faith. The Nomination Form also has the timeline that the ministry will follow when selected to move forward at the March meeting.
The Application Form is to be completed by the ministry representative after a successful nomination from the March meeting. The application is then due for submission 2 weeks prior to the April meeting. The April meeting will also give the ministry an opportunity to present about their mission and plans for the grant at the April Meeting.
